Library incident report

Additional reporting to the University may be required if this incident includes examples of sexual misconduct, discrimination, or related retaliation. Suspected or confirmed sexual misconduct may include sexual harassment, sexual assault, stalking, or relationship violence. Suspected or confirmed discrimination may include an adverse action taken because of a protected characteristic, a denial of reasonable accommodations, or discriminatory harassment. Libraries HR will initiate this reporting and you may be contacted by HR or Equal Opportunity & Title IX office representatives. This report is sent to a number of people across the Libraries who need to be aware of incidents happening in the various buildings. If you’re reporting internal issues or you'd like a more confidential place to report, please contact the Libraries HR Director, Jessica McCann ([email protected] or 612-624-4362). You can also report directly to the Equal Opportunity & Title IX office.

What emergency services responded?
Explain what happened and how you or others (e.g., facilities, management, security, etc.) responded.

Describe people in as much detail as you are able. Consider:

  • name and contact info (e.g., email, phone number)
  • relative age
  • gender expression
  • relative height and build
  • hair color, length, and type
  • clothing
  • any distinctive features (scars, tattoos, etc.)
